From January 2016 to December 2020, the Fudan University Shanghai Cancer Center, China, performed pulmonary resection on 14,104 patients with lung cancer or pulmonary nodules. Cases of re-exploration for bleeding episodes were considered, and the interplay between post-operative hemorrhage and patient characteristics was investigated. To decrease re-exploration procedures related to bleeding, a protocol was further developed and implemented at our center.
Re-exploration due to bleeding affected 85 (0.60%) of the 14,104 patients. Surgical incisions (20, 2353%), parietal pleura (20, 2353%), bronchial arteries (14, 1647%), lung parenchyma (13, 1529%), pulmonary vessels (5, 588%), and infrequent instances of bleeding from unusual locations were among the causes of postoperative bleeding. Postoperative bleeding presented with diverse patterns. Video-assisted thoracoscopic surgery (VATS) demonstrated a significantly lower bleeding rate than open thoracotomy, exhibiting a difference of 127% versus 0.34% respectively (p<0.00001). Significant variations were observed in the bleeding rates following pneumonectomy, lobectomy, segmentectomy, and wedge resection procedures (178%, 88%, 46% versus 28%, p<0.00001). All patients were successfully discharged, with the exception of one, who succumbed to respiratory failure. Based on the presented data, a protocol was created to curtail the incidence of bleeding-related re-operations in our institution.
